The 佛羅里達 Medicaid Plans offer a range of benefits along with extras that you may find useful.
Our Medicaid (MMA) plan covers children, families, pregnant women and single adults who may qualify based on income. UnitedHealthcare Health & Home Connection is a coordinated care management program for community-based elderly who are at risk of nursing home placement.

- Region 11: Miami-Dade and Monroe Counties
The Health & Home Connection SMMC LTC Program is available in the following counties:

Talk to your UnitedHealthcare network doctor about getting the flu shot. If you don’t have one, please contact Member Services at 888-716-8787 and a representative can help you find one.
