Search our directory of network providers by selecting one of the links below.
Provider Search
If you choose the “Search for a Provider” link, enter the following information:
- Type of Facility - All Facilities
- Plan Name - UnitedHealthcare Health and Home Connection
- Additional Options- Enter County, State or other information you would like to add to your search
- Click on the “Find Facility” button
Behavioral Health Provider Search
If you choose the “Search for a Behavioral Health Provider” link, enter the following information:
- Search For- Medicaid Clinician Only or All Clinician
- Select a State- FL- 佛羅里達
- Enter One of the Following- City, Zip Code, Clinician Name, or Group Name
- Narrow the Search- Pick one or more item to add to your search
- Click on the “Search” button
