For individuals and families living in Kenosha, Milwaukee, Ozaukee, Racine, Washington or Waukesha counties, dental services like cleanings, checkups and preventive dental work are covered by UnitedHealthcare. 

Call our Member Services Department at 1800-504-9660 or visit myuhcdental.com, for the names of UnitedHealthcare dentists. 

If you live in any other county, you may get dental services from any dentist who will accept your ForwardHealth ID card. Your dental services are provided by the State of Wisconsin, not UnitedHealthcare. If you have any questions, contact the State at 1-800-362-3002.

災害或緊急情況聲明

如果受到總統或州長聲明的災害或緊急情況影響，或者是衛生及公共服務部部長所公佈的公共衛生緊急情況的影響，您可以獲得一些額外的支持協助。

  • A 部分、B 部分和 Supplemental C 部分計劃承保福利須在指定的非合約機構提供（注意：A 部分和 B 部分承保福利必須在 Medicare 認可的機構獲得）；
